Reduced rates on physicals

High School Physicals will be offered on the following dates at ACMC in Granite Falls:

July 23, July 31, August 3, August 7 from 9 a.m. - 4 p.m.  The cost of the exam is $30 and is payable at time of the exam.  Insurance willl not be filed for these exams.  Bring a copy of the MN State High School League Sports Qualifying Exam form signed by a parent or guardian.
