There will be blood pressure checks, door prizes and refreshments served.

  •     Granite Falls Hospital and Manor is pleased to announce that Home Care has been a part of our services for 30 years. Please join us in celebrating on Wednesday, November 7, from 2:00 - 4:00 p.m. at the Professional Building (located just north of the Dairy Queen) in Granite Falls. There will be blood pressure checks, door prizes and refreshments served.
        The goal of the Granite Hospital and Home Care is to maintain continuity of care for the client between the hospital, nursing home, clinic, physician and/or health care setting., Home Care can be provided to individuals of any age, 0 - 100+ years, depending on their health status, and involves in-home professional nursing service.
        Granite Falls Home Care works with a number of physicians and health care facilities throughout the state. A variety of payment options are available – Medicare, Medical Assistance, Veteran’s Benefits, Private insurance, grants, Private Pay, etc. Service is provided to individuals living within a 30-mile radius of Granite Falls.
        Professional assistance may include education, wound care, IV therapy, palliative care, treatment procedures, injections, lab draws, and health and  medication assessments. Services can last anywhere from a few weeks to  several years, depending upon the client’s needs and pay sources.
        Home Health Aide Services include daily living assistance with showering/ bathing, dressing, grooming, transferring/ambulation, eating and  or medication reminders. Homemaker services may include meal preparation, light housekeeping and/ or local errands.
        Granite Falls Hospital Home Care nurses also offer a mother and baby follow-up home visits to all infants born at the Granite Falls Hospital. Other professional services include physical therapy, occupational therapy, and  speech therapy in the home.
        Telecommuting services are available to clients who require the close monitoring of their lood pressure, oxygen level, heart rate and/or weight daily, and questions specific to their health status. This data is received in the home care office daily and reviewed by a nurse who will communicate with the client, and update their doctor with changes.
